NYS-HCCN Quality Improvement Workshop: “Activating Clinical Care & Support Staff to Improve Patients’ Health Outcomes through Medication Adherence.” | June 7th, 2018 | Five Star Bank Flower City Community Room
The New York Statewide Health Center Controlled Network (NYS-HCCN) at CHCANYS was excited to provide this live workshop opportunity to participating health centers. The full day training was conducted by Christine Chim, PharmD, BCACP, Assistant Professor, Clinical Health Professions, St. John's University, College of Pharmacy & Health Sciences. This full-day workshop offered participants the opportunity to identify and address barriers to medication adherence; explore the latest evidence-based strategies; improve upon new and existing protocols and policies; and create seamless workflows to ensure that key members of your care team will play integral roles in the promotion of medication adherence among your patients. In addition to an expert led session, there was some shared learning opportunities and group-based activities to increase participants’ knowledge and confidence around establishing an organizational culture and capacity for medication adherence.

The NYS-HCCN is partnered with Qualis Health to deliver an interactive learning event for member health centers at all levels of development and skill in care coordination and care management. The learning event focused on evidence-based design and implementation strategies for development of effective care coordination and care management systems as a blueprint for health centers to use for improvement of current state systems for these two critical components of a high-performing medical home.
