Collection of data related to Social Determinants of Health (SDH) can positively impact systemic population health improvement as well as engage patients in addressing their non-medical needs by coordination and increasing access to appropriate services. The Protocol for Responding to and Assessing Patient’s Assets, Risks, and Experiences (PRAPARE) is a patient/client engagement tool structured as a standardized set of core social risk screening questions. PRAPARE has been commonly deployed by Community Health Centers nationally and it is embraced by CHCANYS.
Standardized Data Collection Tool: PRAPARE The PRAPARE tool was developed by NACHC in partnership with AAPCHO and OPCA. The PRAPARE assessment tool consists of a set of national core measures as well as a set of optional measures for community priorities. It was informed by research, the experience of existing social risk assessments, and stakeholder engagement. It aligns with national initiatives prioritizing social determinants (e.g., Healthy People 2020), measures proposed under the next stage of Meaningful Use, clinical coding under ICD-10, and health centers’ Uniform Data System (UDS). NACHC PRAPARE Implementation & Action Toolkit The PRAPARE Implementation Toolkit is designed to provide interested users with the resources, best practices, and lessons learned to guide implementation, data collection, and responses to social determinant needs.
