Clinical Excellence and Innovation

CHCANYS statewide HIT and performance improvement initiatives support New York FQHCs in delivering high quality, patient-centered, cost-effective primary health care for medically underserved populations. CHCANYS’ Center for Primary Care Informatics (CPCI), a robust cloud-based data reporting and analytics solution, is the centerpiece and driver of our performance improvement support to health centers.

Clinical Excellence & Innovation (CEI) is home to the New York Statewide Health Center Controlled Network (HCCN), a collaborative network of 43 diverse FQHCs from across the state working towards strengthening quality of care and improving patient health outcomes. Our work is funded by several prestigious federally-funded practice transformation initiatives and reflects successful partnerships at the local, state and federal level.

Individualized Practice Coaching
Our expert certified practice facilitators provide onsite support to multidisciplinary practice teams focusing on the individual priorities and strategic needs of each FQHC and the specific communities and patient populations served. To strengthen and demonstrate the value of the FQHC model our customized support:

  • Facilitates success in value-based healthcare and payment arrangements by disseminating evidence-based practices to maximize efficiency and performance in metrics related to prevention, chronic disease management, and appropriate utilization.
  • Drives operational excellence in maximizing use of the Health IT, including the CPCI, implementation and Meaningful Use of electronic health records and health information exchange to strengthen quality improvement capacity, data quality and integration, population health management, care management, team-based care, patient engagement, and satisfaction and Patient Centered Medical Home recognition.
  • Promotes integration with the Medical Neighborhood by building community linkages and partnerships and preparing FQHCs to practice according to advanced primary care models while addressing social determinants of health.

Group Training and Peer Learning
Industry expert trainings and collaborative peer-driven learning sessions are developed in anticipation of changes in the health care landscape and emerging needs of health centers. These educational and networking offerings are specifically tailored to an FQHC audience in the New York State context, multimodal (in person, webinar etc.), support sharing of promising practices, and provide access to timely content and industry experts.

Committee and Workgroup Collaborations
CHCANYS regularly convenes FQHC leaders statewide to promote collaboration and dissemination of up-to-date information and resources to expand health center capacity to meet the challenges and opportunities of a rapidly changing health care environment. Committees and workgroups include:

  • HCCN Clinical and HIT Committee
  • CHCANYS Board Clinical and Quality Committee
  • Clinical subcommittees: Behavioral Health, Sexual and Reproductive Health, HIV/HCV/STI, Oral Health, and Nursing

Advocacy and Consultation
Our expert staff serve on local, state, and national committees, workgroups and coalitions where we promote the value and impact of FQHCs and advocate to position FQHCs for success. We develop innovative toolkits and resources to support practice transformation and synthesize relevant policy and practice content into actionable guidance, while providing consultation regarding implementation.
