Health centers are invited to submit letters of support for an application by the NYU School of Medicine to the National Institutes of Health (NIH) to fund a CHCANYS quality improvement program designed to reduce risk factors for cardiovascular disease (CVD) and improve CVD-related outcomes. In brief, the project objectives are to:
improve performance on clinical outcome measures relating to hypertension, diabetes and hyperlipidemia; and
increase adherence to evidence-based guidelines for screening and management of hypertension, diabetes and hyperlipidemia.
The project will test two strategies to support optimal use of EHR and Center for Primary Care Informatics (CPCI) functionality to achieve these objectives. We are seeking 24 health centers to form two cohorts. For both cohorts, the CPCI will provide reports and analytic tools to measure performance and provide feedback to providers. For one cohort, the technical assistance strategy will also include practice facilitation (in-person and telephone coaching and technical assistance). The cohorts will roll out over a 5-year period, with project duration not to exceed 1 year for each health center. The health centers in the cohort receiving coaching will also participate in bimonthly collaborative calls, webinars with subject matter experts and other activities facilitated by CHCANYS and other members of the project team. Our colleagues at the NYU School of Medicine will be submitting the application to NIH in January. If funded, the project would likely begin in early 2014.
Funding will help defray annual subscription costs to the CHCANYS data warehouse, the Center for Primary Care Informatics (up to $5,000) and will support CHCANYS’ Health IT and Quality Improvement Programs providing technical assistance. Click here to read more about the project.
