NYS Health Foundation Hosts "A Conversation With Ed Wagner" On September 10, 2009, NYS Health Foundation hosted "A Conversation with Ed Wagner," Director of the MacColl Institute for Healthcare Innovation at the Center for Health Studies' Group Health Cooperative, and creator of the widely used Chronic Care Model. Health care providers, community leaders, and several of the NYSHF's Diabetes Campaign partners, including CHCANYS, attended the event. NYS Health Foundation Launches "Diabetes Clinical Improvement Network" On September 21, NYS Health announced the launch of a Diabetes Clinical Improvement Network as part of its New York State Diabetes Campaign to reverse the diabetes epidemic. For the first time, the Healthcare Association of New York State, Community Health Care Association of New York State, New York Chapter of the American College of Physicians and the New York State Academy of Family Physicians - representing more than 11,000 primary care providers-are joining forces to improve outcomes for New Yorkers with diabetes. 2009 Training Institute on Trauma-Affected Women, Children, and Families In June 2009, HHS' Strategy Group on Trauma-Affected Women, Children, and Families with HHS hosted a Training Institute that explored the unique problems and innovative, evidence-based solutions for trauma-affected women, children, and families and promoted ‘trauma-informed care' across public health, mental health, and social service systems. FREE Train-the-Trainer Event in Buffalo HHS's Office on Women's Health launched The Heart Truth® Campaign which educates and trains community members and health care providers across the country about all aspects of heart disease in women.
OWH and NHLBI are bringing The Heart Truth Champions training to Buffalo, NY on December 10, 2009 and are recruiting 15 women (first come, first serve) who are involved in health education and/or community outreach programs to participate in this FREE train-the-trainer event!! The training will equip women with information about their #1 killer - heart disease - and will provide them with free tools and educational materials to take back to their communities to educate others.
