CHCANYS Awarded New NYCT Grant CHCANYS is excited to announce that it has been awarded a two-year grant from The New York Community Trust to help community health centers add social determinants of health (SDH) to newly created value-based payment systems. This new funding provides CHCANYS the opportunity to provide community health centers additional training and technical assistance with capturing and addressing SDH in efforts to achieve better population health outcomes. The training will build upon prior NYCT-funded efforts to increase screening and use of data analytics to identify and address population needs.
