CHCANYS Offers data services to FQHCs who are considering participation in collanborative DSRIP proposals. Supporting the sustainable and collaborative growth of health centers is central to the mission of CHCANYS, and DSRIP offers an important opportunity for FQHCs to better integrate with local health delivery systems and extend the benefits of the FQHC model of care.To support FQHCs in DSRIP planning, we have adopted an expedited application and review process for our assistance.
To request our assistance in DSRIP planning, a health center should print and complete the application found here. The application includes a request form with basic contact information and the Terms of Participation. The health center’s CEO should indicate acceptance of the terms by signing the document in the indicated place.
For expedited review, please email a copy of the request form and the signed Terms of Participation to data@chcanys.org. A member of our staff will respond to your application in a few days. (You may also mail the completed and signed materials to DSRIP SUPPORT, CHCANYS – Suite 601, 90 State Street, Albany, NY, 12207).
Participants in the support program may call 518-434-0767 during regularly scheduled “office hours” (Monday and Wednesday 3:00-4:00 and Friday 2:00-3:00) for ad hoc technical support from CHCANYS staff on DSRIP-related data questions.
