CHCANYS Statewide Health Center Controlled Network
CHCANYS’ New York Statewide Health Center Controlled Network (NYS-HCCN) provides training and technical assistance that supports the adoption, implementation, and utilization of health IT solutions to fifty-one participating FQHCs. Founded in 2012 and supported by continuing grant awards, CHCANYS’ NYS-HCCN leverages a collaborative network of New York health centers to help members better harness the technology necessary to thrive in a value-based care environment. CHCANYS’ Center for Primary Care Informatics (CPCI) is a web-based data reporting and analytics tool that plays a central role in the network’s efforts.
Member Goals
NYS-HCCN has designed a comprehensive training and technical assistance program to ensure participating health centers and their providers have the resources and capacity to:
Enhance patient engagement and reduce provider burden
Support interoperability for care coordination
Use data to enhance value
Advance adoption of telehealth in New York’s FQHCs

The NYS-HCCN provides training and technical assistance through individualized practice facilitation, peer learning calls and workshops, office hours and advisory support from industry experts, and the distribution of best practices across the network.
