Gabriela is part of the Quality and Technology Initiatives Program, supporting community health centers' adoption of innovative practices with the goal of achieving data-driven transformation of care at the patient, provider, organizational and community level. Gabriela comes with a background in health education, promotion programming, and project management for an Article 28, Federally Qualified Health Center. Gabriela also has experience with a large network of community-based organizations in New York City. Gabriela has joined CHCANYS to continue to work toward health care quality improvement and population health management, to assist and facilitate ongoing efforts and management methodologies across diverse health care practices that aim to achieve and sustain quality outcomes measures in accordance to increasing value-based health care.
Gabriela has a BS in Public Health from Rutgers University, Edward Bloustein School of Planning and Public Policy and she is pursuing a Master of Applied Science (MAS) in Population Health Management at the Johns Hopkins Bloomberg School of Public Health.
