The document focuses on those projects identified by New York State for planning considerations among Performing Provider Systems that are relevant to participation by FQHCs.

Reducing Hospital Readmissions: Lessons from a Multi-Hospital Initiative The Preventable Hospital Readmission Initiative was launched by the United Hospital Fund in 2011. Its first stage helped seven New York City hospitals identify the causes of preventable readmissions in their facilities; in its second stage, four of those hospitals tested and implemented specific interventions to address those causes. This report gives an overview of the initiative: how it was run, results from its two stages, and lessons learned from its various findings and interventions.
