The New York State Health Foundation (NYSHealth) selected CHCANYS as their first clinical partner to participate in a five-year, $35 million campaign, the New York State Diabetes Campaign (here referred to as the Campaign). The goal of the Campaign is to reverse the diabetes epidemic in New York State by focusing on:
improving clinical care,
mobilizing communities, and
promoting policy.
CHCANYS currently strives to meet these goals with New York State’s federally qualified health centers (FQHCs) through the use of the National Committee for Quality Assurance Diabetes Recognition Program (NCQA DRP) and Bridges to Excellence (BTE) Diabetes Care Recognition Program. The NCQA DRP and BTE DCR are voluntary programs for practices to receive recognition for delivering quality diabetes care. Recognized health centers enjoy benefits such as being distinguished as leaders in the community of practice and leverage NCQA DRP data abstraction efforts for incentives such as that received through the Patient-Centered Medical Home (PCMH) Recognition.
CHCANYS supports the NYS Health Foundation’s “Meeting the Mark”. Goals of this initiative include assisting at least 3,000 physicians in achieving NCQA or BTE diabetes recognition and providing $2,500 per physician who achieves diabetes recognition.

To further address the state of diabetes in New York State, the Campaign launched “Half the Care” – a statewide advertising initiative urging physicians to provide more comprehensive care to their patients with diabetes. The campaign features full-page images of doctors split in half to emphasize that patients receive half the care they need and promotes a message that "We can do better."
