CALL FOR PROPOSALS 2012 Proposals are accepted April 16 – June 15, 2012
The Community Health Care Association of New York State is hosting this year’s Statewide Conference and Clinical Forum Wednesday, October 24- Friday, October 26, 2012 at The Saratoga Hilton & City Center in Saratoga Springs, New York.
Background:
The conference will address timely topics by taking an integrated approach to Health Information Technology, finance, policy and clinical quality areas of focus among others. It is also a great opportunity to learn and share health center best practices, programs and services, and emerging hot topics on the federal and state level that impact health centers.
Areas of Focus for Proposal Submission:
Board Development: HRSA federal requirements; financial guidelines; effective communication strategies; leadership capacity; fundraising;
Special Populations: all topics related to programs and services;
Clinical Quality: QI approaches to care coordination of chronic diseases; use of telehealth services to improve patient outcomes; innovative uses of dashboards to drive QI activities; integrating Triple AIM into QI activities; best practices for chronic health conditions (esp. HIV/AIDS, Hep C, Diabetes, Hypertension); innovative Dental Health Services; innovative Behavioral Health services; best practices on PCMH and Meaningful Use; emerging clinical roles within the PCMH team;
Collaboration: models of collaborations among health centers and others e.g. hospitals,home care, social service providers, community organizations;
Health Information Technology: role in fiscal and operational strategies; Telemedicine: bridging the gap between urban specialists and rural providers; privacy & security; business continuity/disaster recover; the role of Health IT in delivery system reform (Health Home, PCMH, ACO etc.); patient portals; RHIO services; Health Information Exchange; meeting Meaningful Use;
Workforce Development: staff team composition (supporting productivity, efficiency, and quality); workforce approaches to care coordination; shifting/emerging job roles and job categories; training based on health center needs (employer demand training); workforce implications for PCMH;
Infrastructure and Operations: reduction in waiting times; use of technology; re-design; finance strategies; best practice models; strategic planning;
Finance: maximizing reimbursement; effective coding; compliance; methodologies and requirements; strategic financing; and
Other unique programs and services
Target Audiences:
CEOs, CMOs, CIOs, CTOs, COOs, board members;
Finance Directors and fiscal staff;
Senior and Middle Management staff (administrative and operational);
Human Resource Directors, Staff involved in recruitment and retention;
Clinicians of all disciplines (physicians, behavioral health specialists, oral health, mid-level practitioners); and
All those interested in careers in primary care including students and AmeriCorps members.
Proposal submissions will be accepted April 16 – June 15, 2012. Presenter/panelists will be notified by July 1st of their acceptance. All proposals are subject to Committee Review prior to selection.

Proposal Guidelines:
Proposals may be submitted for an individual presenter or a panel;
Centers and/or other organizations may submit up to three (3) proposal ideas;
Additional information regarding credentials and disclosures will be requested for CME application purposes;
Conference organizers may request centers/organizations with a similar topic to form a panel or roundtable to enhance the workshop topic; and travel or accommodations.
Completing the Proposal Form:
Complete all contact information. Indicate name, title, and credentials for yourself and or panelists as they should appear (e.g. Jason Smith, MD, Medical Director, ABC Health Center; Linda Barnes, NP, Quality Improvement Director, Best Health Center, etc.);
Describe your proposed topic as you would recommend it published in 200 words or less. You must include three learning objectives and a write-up as you would like it to be published (note: editing may be necessary due to space needs); and Include an interesting title (not just topic).
