The Community Health Care Association of New York State is hosting this year’s Statewide Conference and Clinical Forum Sunday, October 16- Tuesday, October 18, 2011 at The Saratoga Hilton & City Center in Saratoga Springs, New York.
Background:
The conference will address timely topics by taking an integrated approach to Health Information Technology, finance, policy and clinical quality areas of focus among others. It is also a great opportunity to learn and share health center best practices, programs and services, and emerging hot topics on the federal and state level that impact health centers.
Areas of Focus for Proposal Submission:
Clinical Quality: care management of chronic disease (The Care Model in Hypertensive Patients, Patient Health Navigators and Diabetes Care); clinical impact of HIT development (data warehousing); integrating Triple AIM concepts into QI activities; Emerging CQI Initiatives (HIV/AIDS, Hep C); NY Prescriber Education Program; Dental Health; Behavioral Health;
Collaboration: consider successful collaborations among health centers and others e.g. hospitals, home care, social service providers, community organizations);
Health Information Technology: consider fiscal and operational strategies; Telemedicine: Bridging the Gap between Urban Specialists and Rural Providers; Privacy & Security; Business Continuity/Disaster Recover; The Role of Health IT in Delivery System Reform (ACOs, PCMH, etc.); Patient Portals; RHIO Services; Health Information Exchange; Meeting Meaningful Use;
Workforce Development: Recruitment and Retention (consider unique partnerships and strategies including those for smaller communities or rural areas); Emerging Roles in Healthcare: Residency Training;
Infrastructure and Operations: consider waiting times; use of technology; re-design; finance strategies; best practice models; staffing models; strategic planning;
Board Development: HRSA federal requirements; financial guidelines; effective communication strategies; leadership capacity; fundraising;
Finance: maximizing reimbursement; effective coding; compliance; methodologies and requirements; strategic financing; and
Other unique programs and services
Target Audience:
Senior and Middle Management staff (administrative and operational);
Clinicians of all disciplines (physicians, behavioral health specialists, oral health, mid-level practitioners);
Finance Directors and fiscal staff, CEOs, COOs, board members; and
All those interested in careers in primary care including students and AmeriCorps members.
Proposal submissions will be accepted April 6 – June 10, 2011. Presenter/panelists will be notified by July 1st of their acceptance. All proposals are subject to Committee Review prior to selection.

Proposal Guidelines:
Proposals may be submitted for an individual presenter or a panel;
Centers and/or other organizations may submit up to three (3) proposal ideas;
Conference organizers may request centers/organizations with a similar topic to form a panel or roundtable to enhance the workshop topic; and
CHCANYS will offer the Early Bird registration fee to workshop presenters and panelists. Unfortunately, we are not able to provide honorarium, travel or accommodations.
Completing the Proposal Form:
Complete all contact information. Indicate name, title, and credentials for yourself and or panelists as they should appear (e.g. Jason Smith, MD, Medical Director, ABC Health Center; Linda Barnes, NP, Quality Improvement Director, Best Health Center, etc.);
Describe your proposed topic in 200 words or less. You must include three learning objectives and a write-up as you would like it to be published (note: editing may be necessary due to space needs); and Include an interesting title
