QUALITY IMPROVEMENT

This conference will include presentations by an expert panel of providers who are dedicated to the successful transition of young adults with genetic disease from pediatric to adult-oriented medical care. The talks will be followed by an open discussion with patients who will share their transition experiences. The day will conclude with an informative Q&A session.
