CHCANYS Workshop: Leveraging Data from External Sources to Optimize Practice & EHR Workflows

 

Intended audience: This training session is designed for clinical, operational, and administrative staff, along with health data and quality improvement leads.
 
The goal of the day is to gain a better understanding of how to optimize the external data you are receiving and make it useful for you. Topics will include NYS PCMH Overlaps with Meaningful Use and more, Extending the Primary Care Center into the home: engaging patients through patient portal and patient generated health data, as well as highlighting opportunities to Make the Most of RHIO data.
 
In addition to expert instructor-led training, we will have health center staff members present on promising practices they have had success with around operationalizing their RHIO data, as well as guest speakers from the NYS RHIO’s (or QEs - Qualified Entities). You will have the opportunity to participate in breakout sessions with fellow health centers and experts to discuss successful workflows, while building connections with other helpful resources.
