Join CHCANYS’ NYS Health Center Controlled Network project team on January 9th for an intensive afternoon workshop dedicated to improving diabetes care for your patients through implementation of point-of-care management strategies, recall, and outreach. Our speaker for this event will be Dr. Natalie Levy, MD, Director of NYC Health + Hospitals/Bellevue’s Primary Care Diabetes Program and the Mobile Insulin Titration Intervention Program (MITI). During this 3-hour workshop in New York City, attendees will learn how to:
Create and operationalize registries for more targeted and effective outreach
Communicate patient care gaps and need related to diabetes efficiently to the provider and patient prior to the visit
Optimize the visit when the patient comes through your doors
Engage and motivate patients to manage their diabetes
The target audience for this event includes providers and care team members, staff tasked with creating and generating registry reports for population health management, and staff responsible for patient recall and outreach.
