PCMH Office Hours: Patient-Centered Access and Continuity (AC)

CHCANYS is partnering with Qualis Health to provide monthly PCMH Office Hours with Karen Vest-Taubert, RN, BSN, MBA, Senior Consultant and NCQA PCMH Certified Content Expert. We encourage your staff to take advantage of this opportunity to ask questions regarding your PCMH applications and transitioning to the 2017 PCMH standards. Participants will have a chance to speak to other health center staff about their PCMH-related questions and issues as well. Each monthly session takes a closer look at the PCMH standards and various elements. Karen will also incorporate exercises and discussion topics as time allows. To register, click here.
