Join the Azara team for a webinar on how the PRAPARE social determinants of health (SDOH) are integrated into CPCI. PRAPARE is a tool created by NACHC to help health centers collect the data needed to better understand and act on their patients’ SDOH To register, click here. Additionally, NYC-based FQHCs have the opportunity to receive technical and implementation support from CHCANYS on the PRAPARE SDOH assessment tool beginning in February 2018.
