This webinar will provide an introduction to the PRAPARE tool that helps health centers and other providers collect patient-level data on the social determinants of health. It will walk participants through what the tool is, what is measures, what resources it contains to collect and act on social determinants data, how data can be collected in the EHR and clinical workflow, and how data can be used to transform care and advocate for payment models that sustain integrated care with non-clinical services and community partnerships.
The Protocol for Responding to and Assessing Patients’ Assets, Risks and Experiences (PRAPARE) is a national effort to help health centers and other providers collect and apply data to better understand their patients’ social determinants of health (SDH). PRAPARE is both a standardized patient social risk assessment tool and a process for addressing SDH at both the patient and population levels. Read more about PRAPARE here.
