Clinical Events

CHCANYS has been an active and engaged clinical partner of the New York State Diabetes Campaign.  In efforts of addressing the high prevalence of diabetes in NYS, our goal is to engage all member sites in the Campaign and provide technical assistance in submitting for Diabetes Recognition thus leading to Patient-Centered Medical Home Recognition.  CHCANYS is bringing the campaign to you by hosting a series of regional symposiums:

 Monday, December 13^th 2010 – The Saratoga Hilton, Saratoga Springs, NY – 10:30AM to 3PM

 Symposium topics will cover:

• Best practices in clinical diabetes care at FQHCs in NYS
• Meaningful use of EMR and medical home
• Community resources that address diabetes prevention
