Position Description:

PURPOSE OF THE POSITION

The director of Risk Management is responsible for risk management for all Gotham Health Sites. The director will ensure that NYC H+H/Gotham Health will be in compliance with all risk management and reporting regulations. 

 

AREAS OF RESPONSIBILITIES (All Gotham Health sites)

The incumbent will monitor progress, manage the activities of projects, and will act as the liaison  between internal and external stakeholders and partners to facilitate collaboration and ensure accountability. The incumbent will function as a subject matter expert and as a professional lead in assigned areas. 

 

SUMMARY OF DUTIES AND RESPONSIBILITIES

The director of Risk Management is crucial to ensuring accountability for delivering high quality care and meeting Gotham's safety and patient experience principles. The director will lead risk management activities and assist with quality assurance and systems development to meet on-going requirements from regulatory agencies such as the Health resources and Services Administration, the New York State Department of Health and other similar agencies, as well as compliance with Gotham Health Assurance Program and Performance Improvement Plan. These tasks will include developing, implementing and maintaining a risk management plan for all health services; crucial and timely dissemination of incident and occurrence details and results of root cause analyses; and supervising staff who have a direct role in quality assurance and risk management activities. 

1. Investigate complaints, incidents, and occurrences; and conduct root cause analysis for all reportable incidents and occurrences.
2. Ensure that incident/occurrence reports are disseminated to the appropriate persons/entities in a timely manner.
3. Present, in person and in writing, risk management activities, employee and patient occurrences, and the results of root cause analysis to the Quality Assurance committees of the Gotham Health and NYC H+H Boards of Directors.
4. Create and monitor policies that improve both patient care and employee safety and adhere to standards and guidelines of NYC H+H and the city, state and federal agencies.
5. Educate and train staff about potential risks.
6. Work with the hospital's legal counsel as appropriate.
7. Collect data to identify problems or weaknesses in health center procedures.
8. Coordinate and monitor risk management, quality improvement efforts and plans of corrections.
9. Monitor indicator data across all 58 sites for trends, best practices, and poor performance. Develop improvement plans with site leadership.
10. Monitor patient complaints across all six health centers in order to assist in developing strategies for improving patient satisfaction efforts.
11. Adheres to confidentiality and patient rights in accordance with HIPPA
12. Promotes a positive and pleasant attitude conducive to teamwork aiming to enhance and further the mission, vision, and values of the facility
13. Participates in peer groups and in-services as necessary to keep current with the body of knowledge
14. Assists the Director of Health Care Standards in the creation, implementation, monitoring and reporting of quality assurance and quality improvement activities.
15. Completion of RCAs for all incidents
16. Conduct proactive risk assessment
17. Educate and train staff to identify risk
18. Review legal documents and coordinate staff interviews with outside counsel
19. Sequester documents as needed
20. Sharing harm events and best practices system-wide to prevent similar occurrences at other sites

 



Special Instructions:

QUALIFICATIONS/CERTIFICATION(S)/LICENSE(S)

1. Comprehensive knowledge of Risk Management principles and practices. Proficiency in root cause analysis, required.
2. Working knowledge of FQHC program and its requirements is preferred.
3. The ability to communicate effectively with all levels of staff from senior leadership to line workers in order to coach and assist. 
4. Strong interpersonal, written and verbal communication skills, with the ability to deliver complex information in a manner that is understandable.
5. The ability to structure and manage group processes.
6. Attention to detail and analytical skill.
7. Ability to maintain a calm, professional demeanor under pressure.
8. Excellent planning and organizational skills including the ability to set time lines and meet deadlines.
9. Systemic problem-solver able to lead and guide a team to implement solutions to solve root cause problems.
10. Capacity to understand team dynamics and the use of developmental tools to promote collective change.

 

COMPUTER PROGRAMS/SOFTWARE OPERATED

Proficiency in personal computer and desktop applications, in particular a command of Microsoft Office applications, Excel, Access, PowerPoint (all required).

 

At least five years of progressively responsible experience in risk management, preferably in Ambulatory care.
