Position Description:

We are currently recruiting for a Social Worker Adult Day Health Care Program.
 
Adult Day Health Care Program Behavioral Health Services
  • Conduct participant’s psycho-social intake assessment prior to program enrollment
  • Conduct ongoing, scheduled assessments for the purpose of re-evaluation on a semi-annual or as needed basis, using evidence based behavioral assessment tools (i.e. PHQ-9, DAST)
  • Utilizing assessment information and in coordination with the participant, create meaningful and achievable goals for the participant’s care plan
  • Provide group-based processing programs for participants (e.g. Dialectic Behavioral Theory, Anger Management, Substance Use Recovery/Maintenance, etc.)
  • Meet with participants for individual sessions for on an as-needed basis or as indicated in their comprehensive care plan
  • Maintain accurate and timely documentation (on paper forms and/or within the EMR) for each participant for assessments, individual sessions, and group-based sessions
  • Incorporate and utilize trauma-informed care and harm reduction philosophies within the scope of group, individual and care planning services
  • Attend Adult Day Health Care Program administrative team meetings as scheduled
  • Attend Adult Day Health Care Program clinical meetings as scheduled
Care Coordination/Care Management services
  • Coordinate case management services to a participant caseload of up to approximately twenty-five (25) participants.
  • Orient new participants to service(s) of the program.
  • Facilitate compilation of interdisciplinary participant-driven comprehensive care plan for each participant
  • Review comprehensive care plan with each participant and determine appropriate schedule for group attendance on a monthly basis.
  • Assess for client-specific barriers that may hinder progression of the care plan.
  • Coordinate joint services and treatment planning, assessment and reassessment for participants receiving services from multiple providers and organizations.
  • Coordinate case conferencing with other service providers to clarify and define each provider’s goals and objectives.
  • Provide linkage, counseling, and referrals for participants around useful risk reduction and safer sex information and current HIV/AIDS treatment care. Refer to risk reduction and treatment adherence groups as appropriate.
  • Promote self-management and support for each patient
  • Participates in discharge planning activities when appropriate.
  • Outreach to individuals who are assessed as frequent absenteeism from the program.
Crisis Intervention Services
  • Facilitate participants’ creation of an Individualized Crisis Intervention Plan
  • Provide crisis intervention and/or de-escalation services on an as needed basis, during program operation

  • Requires the ability and commitment to respect and support inclusiveness and diversity including but not limited to individuals of different backgrounds, cultures, races, ages, sexual orientations, gender identities or expressions, experiences, opinions, etc.
  • Requires individual demonstration of commitment to the One Trillium behaviors and business impacts and modeling them in the organization.
  • Responsible for maintaining confidentiality of all patient, client, employee, protected and proprietary information.
  • Employees are accountable for meeting the performance standards of their departments and must participate as requested in compliance audits, process improvement and quality improvement plans.

Qualifications
Masters in Social Work or Mental Health Counseling, and licensed by NYS. Experience in HIV, Substance Abuse and/or Mental Health preferred. Ability to speak Spanish and/or ASL preferred. Must be accustomed to diversity.
