Position Description:

We are currently recruiting for a Community Outreach Specialist Rural (Chemung & Schuyler Counties) Bath Office.
 
Outreach – includes frequent nights & weekends- focused in rural Chemung and Schuyler Counties.
  • In accordance with policy and procedure, as well as employee conduct and workplace rules, participate in outreach at appropriate events, community spaces, bars, nightclubs providing information on HIV prevention.
  • Pro-actively identify and solicit location and event opportunities for outreach to occur.
  • Engage individuals in conversations surrounding sexual and substance using behaviors in a non-judgmental manner.
  • Provide risk reduction supplies and PrEP, PEP, and Treatment as Prevention information and education to targeted individuals.
  • Ensure educational resources, prevention, and collateral supplies are available to targeted outreach venues.
  • Participate in required training opportunities and attend regularly scheduled team meetings.
  • Demonstrate developing knowledge of HIV and STIs and their transmission, HIV treatment, as well as PEP and PrEP.
  • Compile and report outreach data/demographics as required.
Community Based HIV Testing – includes frequent nights & weekends
  • Liaise between Trillium Health’s Rochester clinics, referral locations and other outreach teams to facilitate client referrals for HIV treatment, PrEP/PEP services and primary care.
  • Provide active referrals to testing services at Trillium Health when providing outreach or, conduct the HIV test when needed to expedite referrals and linkage to HIV rapid start treatment, primary care and/or PrEP/PEP services.
  • Active referrals include escorting patients to appointments or telehealth appointments via accompanying them on public transportation, arranging transportation, or transporting them in your car.
  • Responsible for providing patients with STI and HCV testing or linkage to testing.
  • Complete confirmatory blood draws, required paperwork and transfer of the sample to the appropriate lab for individuals with reactive HIV rapid tests.
  • Link people with a reactive HIV test to rapid start HIV treatment through traditional or telehealth appointments.
  • Link people who are living with the virus to primary care, HIV treatment, care management, and any other appropriate clinical and supportive service.
  • Provide facilitation of evidence-based interventions, as appropriate, during testing encounters for community events.
  • Demonstrate developing knowledge of HIV/AIDS, End the Epidemic by 2020 including ART and PrEP/PEP, health disparities and other health-related issues.
Social Media – includes frequent nights & weekends
  • Conduct Know your status/HIV testing/ Undectable = Untrasmittable outreach and engagement online. Provide community feedback and engagement as appropriate.
  • Proactively source pages and sites to engage (social media pages and groups, personal pages, blogs, etc.).
  • Engage in outreach activities via smartphone apps.
  • Engage communities through online sites, such as ‘hook up’ applications, sites, and chat portals.
  • Proactively share content relevant to the work at Trillium Health including primary care, ART, and PrEP.
Service Navigation and Linkage to Care
  • Link those interested and/or at high risk for HIV acquisition with PrEP/PEP services team at either Trillium Health or an affiliated medical facility or other agency performing PrEP/PEP services, sexually transmitted infections screening and treatment, HIV specialty care, and primary care as needed.
  • Increase individuals’ awareness of and access to Trillium Health’s services, providing active referrals whenever possible.
Miscellaneous
  • Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
  • Responsible for maintaining confidentiality of all patient, clients, proprietary, and protected information.
  • Must have a reliable vehicle and meet the agency driving policy requirements for this position, including a valid NYS driver’s license and proof of required automobile insurance.
  • Must pass on-the-job training to do basic phlebotomy in order to do confirmatory blood sample draws for anyone with a reactive rapid HIV test.
  • Employees are accountable for meeting performance standards of their departments. They participate in compliance audits and quality improvement plans.
  • Other specific duties as assigned.
Qualifications
Bachelor’s degree in human services or related field, or an Associate’s degree and three years outreach experience with targeted population preferred. Requires a minimum of a High School diploma or equivalent plus five years of related healthcare or outreach experience.
