Open Door is seeking a Clinical Nurse Coordinator (RN) for its School Based Health Centers in Ossining and Port Chester, NY. The Clinical Nurse Coordinator will promote and restore patient’s health by completing the nursing process, maintaining documentation, ensuring that the SBHCs are equipped to achieve their patient care objectives, and collaborating with the care teams to provide comprehensive quality care and support to patients and families. He/she will participate in cross-functional care teams. All Open Door Family Medical Centers are located in Westchester and Putnam counties located just North of New York City, which offer excellent schools, easy access to a multitude of outdoor activities, and superb transportation connections to New York City. 


Responsibilities: 

  

  • Responsible for monitoring and maintaining the environment of care of all seven SBHCs, and preparing documentation in compliance with JCAHO standards, SBHC guidelines, and applicable city, state and federal laws.
     
  • Responsible for medication management and infection control in all seven SBHCs, including documentation, inventory, reconciliation and ordering; storage/refrigeration.
     
  • Responsible for preparation and submission of month reports. 
     
  • Supervises the Registered Nurse or Licensed Practical Nurse, and the Supervisor – Patient Care Services (PCS) who is in turn responsible for supervising the Patient Care Technicians in all seven SBHC sites.
     
  • Maintains positive, supportive communication with direct reports, in person and via phone/email, and works to cultivate a strong and collaborative team.
     
  • Assists with scheduling and follow-up of missed appointments, and referrals. 
     
  • Participates in training, orientation, competency assessment and education of new employees.
     
  • Participates in periodic outreach events to promote health and enroll new patients in the School Based Health Centers.
     
  • Participates in patient care, particularly during flu season, assessing patient’s needs, performing and recording vitals, administering vaccinations/medication in accordance with the provider’s orders, performing laboratory procedures, and documenting care in the patient’s medical record and in NYSIIS, following ACIP guidelines.

    Requirements: 
  • Associate RN is required 
  • B.S.N. is preferred 
  • Current NYS RN license 
  • 1 year experience in community health 
  • 2 year’s previous experience in nursing preferred 
  • Bilingual (Spanish) Preferred 
  • Current CPR Certification 
  • Outstanding written and verbal communications skills 
  • Ability to work in a culturally diverse patient and staff environment 
  • Ability to work in a collaborative team-based care environment 
  • Highly organized, flexible, and self-directed, able to multi-task 
  • Responsive and comfortable working in a fast paced environment 
  • Excellent judgment skills and ability to prioritize tasks
