Open Door Family Medical Centers has been serving low-income, under-served residents and those lacking access to health care in the communities of Westchester and Putnam counties for more than 40 years, providing high quality primary health care and human services at affordable prices. Our family medical services include primary medical care, dentistry, mental health, optometry, podiatry, and women’s health in Ossining, Sleepy Hollow, Port Chester, Mamaroneck, Mount Kisco, Brewster, and Saugerties NY.

Open Door is seeking a Registered Nurse (RN) for its School Based Health Centers in Ossining and Port Chester, NY. The Registered Nurse will promote and restore patient’s health by completing the nursing process, maintaining documentation, ensuring that the SBHCs are equipped to achieve their patient care objectives, and collaborating with the care teams to provide comprehensive quality care and support to patients and families. He/she will participate in cross-functional care teams. All Open Door Family Medical Centers are located in Westchester and Putnam counties located just North of New York City, which offer excellent schools, easy access to a multitude of outdoor activities, and superb transportation connections to New York City.

Responsibilities:

  • Performs and accurately records screening tests (blood pressure, temperature, pulse, weight, EKG, vision, hearing) to patients of pediatric and adolescent age groups.
  • Encourages patients to continue under medical supervision and aid them to carry out medical and social work recommendations.
  • Participates in outreach events to promote health and enroll patients in the SBHC.
  • Establishes a compassionate environment by providing emotional and psychological support to patients, friends, and families.
  • Identifies individual and community problems and makes appropriate referrals in coordination with team members.
  • Participates in patient counseling and education activities related to primary health care, HIV/STD prevention and treatment, child/adult health maintenance, perinatal and family planning.
  • Documents necessary information in patient’s electronic medical record.
  • Updates the practice management information system with all necessary clinical information for each patient prior to close of business day.
  • Prepares and maintains documentation in compliance with Joint Commission standards and applicable city, state and federal laws.
  • Administers medications, vaccinations and treatments as authorized by the provider.
  • Performs simple laboratory procedures including but not limited to rapid strep test, urinalysis, pregnancy testing, and tuberculosis screening.
  • Encourages preventative care to patients of all ages.
  • Assists health team in appointment scheduling and follow-up of missed appointments and non-complaint patients by telephone and mail.
  • Maintains patient confidentiality at all times as per HIPAA and Open Door policies and procedures.
  • Reviews patient record one-day prior to scheduled visit to ensure chart preparedness.
  • Maintains current up to date immunization record through NYSIIS.
  • Follows current ACIP guidelines.
  • Participates in the storage and maintenance of medications and immunizations.


Requirements:

  • Associate RN is preferred, highly experienced LPN considered"
  • B.S.N. is preferred
  • Current NYS RN license
  • 1 year experience in community health preferred
  • 2 year’s previous experience in nursing preferred
  • Bilingual (Spanish) Preferred
  • Current CPR Certification
  • Outstanding written and verbal communications skills
  • Ability to work in a culturally diverse patient and staff environment
  • Ability to work in a collaborative team-based care environment
  • Highly organized, flexible, and self-directed, able to multi-task
  • Responsive and comfortable working in a fast paced environment
  • Excellent judgment skills and ability to prioritize tasks
