Open Door is seeking multiple candidates for a Full-time bilingual English-Spanish speaking Behavioral Health Integration Specialist, LMSW for its Family Medical Center in Sleepy Hollow, NY. The Behavioral Health Integration Specialist will report to the Director of Behavioral Health, based in Ossining, NY. All Open Door Family Medical Centers are located in Westchester and Putnam Counties located just north of New York City, which offer excellent schools, easy access to a multitude of outdoor activities, and superb transportation connections to New York City. The dedicated individual will work closely with the primary health care team to provide integrated behavioral health care to improve the patients’ medical outcomes. The Masters’ level clinician will possess outstanding clinical skills and work well with the interdisciplinary health team.
RESPONSIBILITIES:
Identify, screen, and triage patients for intervention daily
Provide brief intervention treatment to patient and/or family
Conduct screenings using appropriate screening tools to identify school readiness and monitor developmental milestones
Document symptoms, problems, screening results, and interventions provided
Develop treatment plan, goals, and objectives with patient
Provide and facilitate communication with treatment care team providers, both medical and behavioral health, and within the community
Incorporate health and wellness education and referrals for preventive care
Respond to and incorporate physical health concerns into brief treatment interventions
Provide crisis intervention and Warm Hand Off assessments to behavioral health staff
Participate in weekly group and individual supervision, and clinical training seminars
REQUIREMENTS:
Masters in Social Work, in Marriage and Family Therapy, or in Mental Health Counseling
Current New York State registration and license
Bilingual in English and Spanish
Recent clinical experience or training
Excellent clinical knowledge with mentally ill and substance-abusing population
Knowledge of evidenced-based treatment modalities and crisis management
Excellent written and verbal communication skills
Experience in working with lower socio-economic and multicultural populations
Ability to work collaboratively with other staff, treatment providers, and community agencies
Highly organized, self-directed, able to multi-task in a fast-paced environment
Strong computer skills including working knowledge in Word and Excel, as well as capacity to navigate and enhance proficiency in these programs
Patient-centric with empathy, excellent active listening and communication skills, engaging and efficient
Passionate about providing high quality care to low-income, under-served residents and those lacking access to health care
