Position Description:

We are currently recruiting for a Peer Navigator MSM Mocha Center, Part Time.

 

Advocacy and Support

  • Through one-on-one interactions and in groups, will promote patient/clients’ engagement in health care and provide opportunities for education, skill-building, and emotional support in a respectful setting.
  • Provide education and information to support and help patient/clients access health information.
  • Provide ongoing support and advocacy to patients/clients with the intent to help enhance or develop coping skills, reduce feelings of social isolation, and increase self-determination and self-advocacy that can result in improved quality of life.
  • Have an understanding and commitment to core ethical principles and ethics-based boundaries of work, including appropriate and inappropriate sharing of personal information, and the confidentiality of information gathered during the delivery of peer support services.
  • Work in collaboration with supervisors and other members of the interdisciplinary team in order to address concerns related to patient/client needs.
  • Work with staff members to schedule appointments on behalf of patients/clients and facilitate appointment-keeping.
  • Build relationships with patients/clients and all members of the interdisciplinary team to support continuity of care and promote health.
  • Assist patients/clients by linking them to needed resources
  • Support patients/clients with adherence to treatment recommendations.
  • Document each patient/client interaction comprehensively and concisely. All documentation will be submitted to supervisor within the designated time frame.
  • Adhere to all government and funder regulations.
  • Participate in case conferences, program, and staff meetings when applicable.
  • Educate patients/clients about services available at Trillium Health.

 

Linkage and Retention

  • Serve as an advocate for newly HIV diagnosed patients/clients and assist in coordinating entry into medical care and care management.
  • Link hard-to-reach patients/clients to needed services by providing outreach services to them.
  • Coordinate transportation for patients/clients, attend appointments with clients when appropriate, and conduct home visits as needed.
  • Coordinate and network with community service providers in order to develop and leverage community resources.
  • Work in collaboration with other providers for engaging and retaining patients in care.
  • Demonstrate developing knowledge of community resources.

 

Miscellaneous

  • Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
  • Responsible for maintaining confidentiality of all patient, client, proprietary, and protected information.
  • Regional travel and variable hours required.
  • Must have a reliable vehicle and meet the agency driving policy requirements for this position, including a valid NYS driver’s license and proof of required automobile insurance.
  • Employees are accountable for meeting performance standards of their departments. They participate in compliance audits and quality improvement plans.
  • Other specific duties as assigned.

 

Qualifications

 

High School Diploma or equivalent preferred.  Experience with target population and fluency in Spanish preferred.  Knowledge of HIV, prevention and counseling preferred
