JOB SUMMARY
The Program Manager is responsible for developing policies, processes and guidelines for the Care Coordination program. This position is responsible for recruitment, hire, and supervision of all frontline staff. The Program Manager oversees daily operations to make sure the program is running smoothly and ensure Care Coordinators and Patient Navigators have the support they need. The Program Manager oversees all monitoring, reporting and quality management and improvement activities for the program. The Program Manager will manage a small case load of the program’s most disenfranchised patients.
JOB RESPONSIBILITIES
Essential Duties & Functions:
Oversee the planning, initiation and implementation of RWPA Care Coordination at MHHC;
- Conduct Psychotherapy for as small case load of Ryan White Part A Part A patients who are vulnerable and in need of trauma-informed counseling;
- Provide on-going direction and supervision of RWPA Care Coordination Services and shared;
- Be well-versed in the project scopes or work, goals and deliverables of the RWPA Care Coordination grant and develop project work plans to ensure program deliverables are met; Develop and/or update, as needed RWPA case management policies consistent with both organizational and donor funding requirements; On a regularly scheduled basis conduct and lead RWPA team program meetings & maintain meeting minutes;
- Participate and take on a leading role in RWPA quality assurance and performance improvement activities;
- In collaboration with MHHC Finance Department & Program Administrator, monitor and track the RWPA budgets to ensure proper spend down of grant funds for allowable expenses; develop budget outlines;

JOB SPECIFICATIONS

1.

EDUCATION & EXPERIENCE
|
LCSW required. At least three years’ work experience in program development and program management preferably in HIV Services; Experience writing reports and managing budgets;

INTERPERSONAL SKILLS
|
Ability to foster good working relationships with clinical and administrative staff. Clearly communicate with individuals and groups of end users, effectively prioritize and communicate concerns to management; ability to foster and build healthy relationship with internal and external stakeholders including Public Health Solutions, Department of Health and executive staff. Interact effectively in a team environment. Ability to speak effectively in group or individual settings. Bilingual (Spanish) desirable. Ability to write and complete routine reports and correspondence Ability to write, read and interpret documents such as procedure manuals.

LANGUAGE SKILLS
|
Strong oral and written communication skills a MUST. Able to clearly explain technical concepts to non-technical individuals. Translate clinical users’ template and report requests into computer programming specifications. Document in procedure manuals. Type/use keyboard accurately and with reasonable speed.

MATHEMATICAL SKILLS
|
Ability to add, subtract, multiple, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Ability to draw and interpret various graphs.
