Morris Heights Health Center (MHHC) is seeking dynamic and energetic Family Physician (s) to join our expanding outpatient clinics in the Bronx. The incumbent would need to support and lead process improvements in access, patient care management, patient/family self-care support and clinical quality performance evaluations.

JOB RESPONSIBILITIES:

• Provide high quality primary care to patients encompassing adult, pediatric and GYN Care.
• Provide comprehensive patient care and establish a family partnership whenever possible to ensure the needs of the patients are met on an ongoing basis
• Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care
• Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance.
• Consult with appropriate medical specialist when necessary; refer patient to subspecialists when indicated
• Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions
• Perform other related duties as assigned

REQUIRED QUALIFICATIONS:

• Medical degree from a medical college approved by the American Medical Association
• Completed internship and residency training from an approved medical facility
• Board certified or board eligible by specialty board

PREFERRED QUALIFICATIONS:

• Focused on collaborative team-based care
• Comfortable working in a high accountability environment and receiving and providing constructive feedback
• Passionate about providing high quality care to low-income, underserved residents and those lacking access to health care
• Spanish and Bengali language skills are very useful
