Position Description:

Summary:

To act as part of an interdisciplinary team providing assessment of WeCARE clients to determine their ability to perform essential work functions.  Participants requiring consultations will be referred as per the Phase I Physician’s orders.  The Phase I Physician is responsible for the evaluation and disposition of his or her panel of participants and for related administrative activities.

Requirements:

  • Assesses each case presented and provides affirmation of the disposition upon completion of visit.
  • Reviews bio-psychosocial and specialty assessments, as needed, to evaluate client.
  • Reviews Phase 2 recommendations and reports from specialists participating in the program.
  • Performs all duties with focus on individual participant disabilities and strengths to develop targeted goals and to maximize individual self-sufficiencies, taking into account the context of the participant’s strengths and limitations.
  • Provides staff consultation on medical issues.  If discrepancy, Medical Director will make final decision.
  • Assists Medical Director and Medical Unit Manager in ensuring contractual compliance.
  • Ensures delivery of quality services to participants in accordance with program objectives.
  • Assists in training activities, as needed.
  • Maintains documentation through computer-based programs.  Enters assessment information in EMR on timely basis.
  • Is trained in and responds to onsite emergencies.

May be assigned to other tasks or duties related to job skills and responsibilities, including coverage for other WeCARE providers.

Education/Qualifications:

  • Degree from an accredited Medical School
  • Completion of an accredited Family Practice, Internal Medicine or Rehabilitation Medicine Residency Program                                                                            Demonstrated two (2) years’ experience in a clinical setting
  • Knowledge of New York State Disability System preferred
  • Specialized training and prior experience with the disabled population preferred
  • Demonstrated interpersonal, communication, and organizational skills
  • Demonstrated basic computer skills
  • Demonstrates an interest in Community Medicine and the mission of the Institute for Family Health
  • Demonstrates good leadership and group dynamic skills
  • NY State Medical License
  • Board certified in Family Practice, Internal Medicine or Rehabilitation Medicine
  • Current BCLS
