Position Description:

Open Door Family Medical Centers has been serving low-income, under-served residents and those lacking access to health care in the communities of Westchester and Putnam counties for more than 40 years, providing high quality primary health care and human services at affordable prices. 

This year, Open Door will be the medical home for more than 57,000 patients, providing more than 300,000 visits. Our family medical services include primary medical care, dentistry, mental health, optometry, podiatry, and women’s health in Ossining, Sleepy Hollow, Port Chester, Mount Kisco, and Brewster, NY.

Open Door is seeking a Patient Advocate for its Family Medical Center in Ossining, NY. The Patient Advocate will report to the Director and Assistant Director of Special Programs, who are based in Ossining. All Open Door Family Medical Centers are located in Westchester and Putnam counties located just North of New York City, which offer excellent schools, easy access to a multitude of outdoor activities, and superb transportation connections to New York City. 

Responsibilities:

  • Provide chronic disease management education (diabetes, hypertension, asthma, etc.) for the purpose of improving clinical outcomes
  • Discuss self-management goals with the patient, assist him or her in selecting appropriate goals, and create an action plan to achieve them
  • Serve as a liaison between primary care providers and patients to ensure that patients can access specialty medical services, medications, and other crucial aspects of health care
  • Maintain electronic medical records in compliance with Open Door’s standards with high efficiency
  • Assist patients with insurance and billing issues
  • Assess patient circumstances to determine eligibility for any government-sponsored programs such as the NYSDOH Cancer Services Program
  • Make appropriate and timely referrals to concrete services for issues such as a lack of food security, domestic violence, lack of insurance, and illiteracy
  • Educate patients about low-cost pharmaceutical drug programs and facilitate enrollment
  • Ensure performance improvement initiatives are supported with timely and accurate data and consistent actions
  • Represent Open Door in community outreach activities


Requirements:

 

  • Bilingual (English/Spanish) required
  • AS Degree required, BS/BA preferred
  • Proficient in MS Office applications, including Excel and other database systems
  • Experience with community organizations and health education preferred
  • Patient-centric with empathy, excellent active listening and communication skills, engaging and efficient
  • Focused on collaborative team-based care
  • Responsive and comfortable working in a fast-paced, high accountability environment
  • Highly organized, and self-directed, able to multi-task
  • Effective in problem solving
  • Flexible
  • Excellent judgment and ability to prioritize resources where they will be most useful
  • Engaged in learning and continuous quality improvement
  • Outstanding written and verbal communications skills with a culturally diverse patient and staff population
  • Passionate about providing high quality care to low-income, under-served residents and those lacking access to health care
  • Possess experience in verbal and written communications and superior customer service skills
  • Must have valid NYS driving license and daily access to car
