Position Description:
The Patient Engagement Assistant will be serving Open Door Family Medical Center through various internal outreach efforts in order to increase patient satisfaction, learn and document patient feedback, and assist with communication to patients to result in better patient health outcomes.
Duties and responsibilitiesCase Management Duties:
Exhibits respect and sensitivity to cultural and social priorities and emotional needs.
Encourages patients to continue under medical supervision and aid them to carry out medical and behavioral health goals and recommendations.
Assists patients with accessing health, dental, and behavioral health services if not currently engaged in health care.
Performs related work as required.
Outreach Efforts
Act as liaison between patients and the health center by providing health information and education to patients in the waiting areas.
Schedule medical appointments for individuals interested in obtaining new or follow up appointments at Open Door.
Patient Portal:
Create and implement patient portal initiative in Ossining and Port Chester to sign patients up to patient portal and increase utilization of portal usage.
Conducts surveys and questionnaires to specific patients to support Population Health initiatives:
Outreach to patients with high blood pressure who have not been seen at Open Door in the last two years with the goal to understand why they are no longer patients, has their health changed, are they receiving care somewhere else.
NOTE: If they are not receiving care elsewhere, try to get individual back to Open Door care.
Make “Welcome” calls to new patients
Better understand needs and circumstances of patients
Obtain feedback about patient’s first experience with Open Door (identify areas of strength as well as potentially needing improvement)Outreach to patients in 3rd trimester of their pregnancy with the goal of bridging Prenatal to Pediatric Care, have mom choose an Open Door provider as their baby’s doctor
Collect, organize and share patient feedback so that Open Door can better understand what is being done well as well as to identify possible areas of improvementWorks with Outreach Assistant as needed to carry out outreach projects and initiatives
Qualifications
Bi-lingual in English and Spanish.
High School Diploma required. Bachelor’s degree in human service field or social work with two to five years’ experience in community health case management is preferred.
Related community outreach experience is preferred.
Proven ability to function effectively as a member of an interdisciplinary team
Proven ability to work effectively with community partners
Demonstrated interest and ability in individual and community health advocacy.
Working conditions
Frequent contact with patients over the phone and in waiting areas across Open Door sites.
Some exposure to communicable diseases and other conditions common to a health center environment.
Requires adherence to Infection Control Standard Precautions.
Physical requirements
Approximately 50% walking/standing and 50% sitting.
