We are currently recruiting for a LPN Per Diem.
Coordinate Care for Pediatric Patients
Work with the Pediatric health care team to facilitate care
Work with interdisciplinary health care team to keep patients engaged
Actively participate in quality improvement initiatives
Demonstrate positive communication and teamwork within the clinic
Clinical Responsibilities
Provide quality care to patients under the supervision of RN
Collect data (vitals, height, weight) and report to RN
Carry out nursing care plan
Administer medication per medication policy
Demonstrate flexibility between the clinic (receiving/answering phone calls; preparing for patient visits) and floor roles (support flow of patients through clinic)
Complete a minimum of 10 hours of continuing education annually
Practice within the NYS LPN scope of practice and maintain annual competencies
Documentation
Document all patient care clearly, thoroughly, accurately, and timely
Order and maintain inventory of medications/ supplies
Maintain ongoing list of patients’ preventative care needs
Miscellaneous

Qualifications
LPN with current CPR certification. Experience working in a pediatric setting required. The ability to demonstrate teamwork and effective interpersonal relations skills is imperative. Excellent written/oral communication and computer skills are required. Previous experience with an Electronic Health Record is desired.
