Position Description:

We are currently recruiting for a Clinical LPN, Sexual Health.

 

Clinical Responsibilities

  • Provide quality care to patients under the supervision of RN and Provider
  • Collect data (vitals, height, & weight)
  • Carry out nursing care plan
  • Administer medication per medication policy
  • Demonstrate flexibility across tasks including direct patient care, indirect patient care and clinic coordination
  • Participate in the clinic orientation of new nursing staff
  • Complete a minimum of 10 hours of continuing education annually
  • Practice within the NYS LPN scope of practice and maintain annual competencies
  • Empower LPNs and MOAs and provide assistance as needed in support of their QA/QI activities
  • Coordinate Care for Trillium Clients
  • Work with the interdisciplinary health care team to facilitate care
  • Work with interdisciplinary health care team to keep patients engaged
  • Actively participate in quality improvement initiatives
  • Demonstrate positive communication and teamwork within clinic and between departments

 

Nursing Coordination of Sexual Health Care

  • Provide nursing support for patients and providers within the sexual health clinic
  • Participate in QA/QI activities related to HRA
  • Investigate sexual health best practices and as appropriate recommend practice and policy updates that reflect evidence-based standards for excellence in sexual health care.
  • Educate other team members including nurses and providers in sexual health best practices.

 

Documentation

  • Document all clinical care clearly, thoroughly, accurately, and timely
  • Order and maintain inventory of medications/ supplies
  • Maintain ongoing list of patients’ preventative care needs

 

Miscellaneous

  • Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
  • Responsible for maintaining confidentiality of all patient, clients, proprietary, and protected information.
  • Employees are accountable for meeting performance standards of their departments. They participate in compliance audits and quality improvement plans.
  • Other specific duties as assigned.

 

Qualifications

LPN with current CPR certification. Experience working in community-based health care settings is also preferred. The ability to demonstrate teamwork and effective interpersonal relations skills is imperative. Excellent written/oral communication and computer skills are required. Experience working with HIV/AIDS clients is preferred, but not required. Experience in a fast-paced outpatient setting is desired. Previous experience with an Electronic Health Record is desired.
