Position Description:

Job Responsibilities:

 

  •  Perform patient triage
  • Prepare  unit for patient flow by organizing daily huddle
  • Maintain hand hygiene and all infection control practices
  • Coordinates patient care equipment checks for biomedical purposes
  • Document care rendered in the EMR including phone notes
  • Ensures unit staff follow-up abnormal labs and imaging results
  • Documents patient and staff incidents and communicates with the Safety committee
  • Administer immunizations and other medications
  • Provide health teaching and re-enforce instructions to patient from provider
  • Obtain prior approval for medical insurance and prescription re-fills
  • Perform  diagnostic testing including blood sugar measurement and peak flow
  • Administers treatments and assists provider with procedures
  • Orders supplies and equipment for units
  • Monitor and report quality and performance outcomes
  • Translate patient information and communication to the patient
  • Participate in all fire drills, codes of all types
  • Maintain current basic life support (BCLS) status
  • Obtain HIV and Bronx RHIO consents
  • Maintain all patient care equipment in operational order
  • Completes competency reviews and  performance evaluations
  • Communicates with external agencies and regulatory bodies.
  • Coordinates unit meetings with collaboration of lead provider and site administrator
  • Attend unit, department and special meetings as scheduled
  • Provide great customer service as evident by professional dress and respectful communication with patients and coworkers

 

 

 



Special Instructions:

JOB SPECIFICATIONS

Graduate of an accredited school of nursing. Bachelor’s degree in nursing preferred.

 

Minimum of five (5) years clinical nursing experience. Ambulatory care experience preferred.

 

Presents self in professional manner.

Ability to remain clam in difficult situations.

Acts as a role model for nursing staff

 

Possesses time management and organizational skills.

Ability to define problems, collect and analyze data and draw conclusions.

Ability to interpret an extensive variety of technical instructions in mathematical or diagram form.

 

Ability to speak, read, write and communicate effectively.

Ability to prepare and deliver concise written and oral reports

 

Possesses basic computer skills

Able to use Microsoft Word, Excel and Power Point programs
