Care for the Homeless (CFH) is a growing not-for-profit health care and social service agency that serves the homeless population of New York City.  CFH operates Susan’s Place, a homeless residence and medical clinic, and provides medical and social services at thirty shelters, soup kitchens, and drop-in centers

This employee will work as part of the PCMH team with the following essential duties and responsibilities:

  • Provide clinical supervision for new disease / care management initiatives across the CFH system of care
  • Actively participates in the expansion of the telemedicine and virtual visit enhances access model
  • Work as a care manager in the disease management model addressing condition-specific patient education needs and all best practice supported clinical diagnostic interventions in conjunction with PCP
  • Provide clinical education and supervision for medical assistant staff
  • Work collaboratively with case management and health education to effectively coordinate care as directed by the primary care medical staff
  • Implement orders for specific interventions as indicated by the medical staff
  • Reach out and engage homeless people with respect to their health status, substance abuse and social service problems;
  • Assess the physical and psychosocial needs of the patient; 
  • Take a complete history of individual and family members;
  • Make referrals to primary care provider for chronic or episodic care; 
  • Administer immunizations, tuberculosis tests, monitor medication; 
  • Advise and educate clients about their physical and mental health, self-care, prevention strategies, and use of medical resources; 
  • Work in nontraditional environments, e.g., non-clinic settings.

QUALIFICATIONS: 

  • New York State Registered Nurse License. 
  • Commitment to vulnerable and disadvantaged populations.
  • Flexible and able to work in nontraditional setting.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee regularly is required to stand, walk, sit, and to use their hand to finger, handle or feel objects, tools or controls; and reach with hands and arms. The employee frequently lifts patients, stretches and has other physical exertion during patient visits and while moving equipment. Specific vision abilities required by this job include close vision and the ability to adjust focus.

While performing the duties of this position, the employee travels regularly by automobile, subway or bus and is exposed to changing weather conditions. The employee is required to travel to meetings with clients and/or other individuals and to outreach sites. The employee may have to travel and spend time over night in another locale.
