Our Community Living Services Program is currently seeking:

Full Time Nurse Case Managers
(Positions Available in Manhattan, Bronx [2], Brooklyn, and Queens)
  • Coordinate the delivery of routine and specialty health care services for adults with disabilities residing in residential programs
  • Be responsible for performing nursing assessments and diagnoses
  • Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes
  • Act as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers
  • Assign Direct Care staff to perform health care treatments/procedures
  • In conjunction with the Residential Coordinator, supervise AMAP staff and work with the Nursing Educator to ensure all AMAP staff are initially certified and annually re-certified
  • Provide on call coverage via cell phone between the hours of 6:00 am – 6:00 pm, Monday thru Friday and be prepared to go onsite if an on call problem warrants a nurse's visit
  • Requires RN with NYS License and one year of experience working with individuals with developmental disabilities. Bachelor's degree preferred.
Full Time Registered Nurses (RNs) – Evening Shifts
Queens, NY Location (Bayside and Jamaica)
  • Assess patients to determine and identify needs, issues, resources, diagnoses and health care outcomes.
  • Develop, update and evaluate effectiveness of health care plans to address assessed needs and providing direct medical/nursing services while evaluating effectiveness of plans.
  • Oversee implementation of the health care plan by training persons receiving services, family, staff, and/or caregivers.
  • Work collaboratively with all involved personnel in managing patient care from admission through discharge.
  • May accompany persons receiving services to clinics, recreation outings, social outings, etc.
  • Maintain close contact with individual's Day Program Nurse to ensure all medical/nursing needs are met in the Day Program.
  • Advocate for health and accesses hospital and specialty clinic services
  • Provide supervision to Licensed Practical Nurse(s) on shift
  • Requires RN (BSN), NYS License, minimum 2 years' experience working with persons with disabilities, extensive knowledge of pertinent health resources and CPR Certification.
