CHCANYS is excited to announce that its Quality and Technology Initiatives (CEI) department has been selected to participate in Round Two of NACHC’s PRAPARE Train the Trainer Academy. The Academy seeks to build knowledge around the PRAPARE tool, facilitate implementation and spread of PRAPARE, and use of PRAPARE data for state policy and transformation initiatives. The Academy will kick-off in August 2018 at the NACHC Community Health Institute and Expo and will run for 10 months through May 2019. CEI staff members have been invited to participate alongside representatives from two of CHCANYS' member health centers that wish to either implement the PRAPARE tool or build upon an existing PRAPARE workflow to develop a robust strategy to underpin data collection on patients' social determinants of health. Please contact Ava Rose or Mercy Mbogori by July 20th 2018 if your health center has the capacity to identify 1-2 staff to participate in remote and in-person Academy meetings and champion the Academy curriculum at the site level in collaboration with CHCANYS staff. The estimated time burden associated with this project is 5% FTE, for which there is no monetary reimbursement.
This call for participation will ideally be answered by community health centers who are:
able to demonstrate the internal capacity and interest in implementing PRAPARE;
currently in the early stages of implementing PRAPARE;
pursuing the expansion of the use of PRAPARE throughout your health center;
seeking strategies to advance the work you have begun around social determinants of health data;
willing to empower 1-2 staff members to dedicate 5% FTE to implementing/spreading PRAPARE at the site level; and
able to fund travel for identified staff to attend in-person Academy meetings at NACHC CHI (August 2018) and NACHC PCA Conference (November 2018).
