Congratulations to Anthony L. Jordan Health Corporation, Betances Health Center, Cornerstone Family Healthcare, Ryan/Chelsea-Clinton Community Health Center, and the Institute for Family Health for being selected by Health Research, Inc. (HRI) and the NYS Dept. of Health to participate in “Increasing Cancer Screening through Patient Navigation in Targeted New York State Federally Qualified Health Centers, a demonstration project designed to improve cancer screening rates among FQHC patients through the implementation of patient navigation and evidence-based interventions.
Each awardee will receive between $75,000 and $125,000 to support one or two full time equivalent patient navigator/s within the health center. The navigators will work with patients in need of cancer screening, helping to facilitate provider-patient communication, identify resources to help overcome barriers to screening, and ensure that patients attend appointments. The five awardees are all participants in another five-year demonstration project supported by the CDC, HRI and the NYS Dept. of Health that aims to project to improve cancer screening rates across FQHCs through use of the CHCANYS Center for Primary Care Informatics. The new funding will support efforts to sustain and build on gains made through this partnership.
