The NYS Dept. of Health (DOH) has just released a Request for Applications (RFA) for the Statewide Health Care Facility Transformation Program. A total of up to $195 million in grants is available to health care providers to support capital projects that address acute inpatient, outpatient, primary, home care or residential health care services needs in a community. A minimum of $30 million of the total funding is designated for community-based health care providers, including diagnostic and treatment centers, mental health and alcohol and substance abuse treatment clinics, primary care providers, and home care providers.
This set-aside represented the first time the State earmarked funding in the budget specifically for community-based providers and was a big win for community-based healthcare. We encourage all interested health centers to apply. Applications must be submitted through Grants Gateway by 4:00 p.m. on Friday,September 16, 2016. DOH will hold an informational webinar on the RFA in early August. Registration information will be posted on the Grants Gateway when available.
