HRSA has announced a new funding opportunity for health center controlled networks nationwide of up to $1.5 million per year for 3 years. New York FQHCs and designated look-alikes are invited to join the CHCANYS application, due February 1, 2016. CHCANYS’ existing New York Statewide Health Center Controlled Network (NYS-HCCN), established in 2012, includes 35 FQHC members from across the state and is funded through July 31, 2016. If awarded, the new round of funding will start August 1, 2016. Both current NYS-HCCN members and new members are welcome to join the application.
The NYS-HCCN will support health centers’ participation in DSRIP and preparation for value-based payments. Specifically, the purpose of the FY16 HCCN funding opportunity is to support health centers in achieving Meaningful Use of Office of the National Coordinator (ONC)-certified electronic health records (EHRs), adopting technology-enabled quality improvement strategies, and engaging in health information exchange (HIE) to strengthen quality of care and improve patient health outcomes. Network technical assistance services funded by the grant will include training and technical assistance in the following areas:
EHR implementation, optimization and achieving Meaningful Use
Enhanced data quality/validation and reporting capabilities
Health information exchange, care coordination, and Population Health management
Quality improvement (QI), including clinical quality improvement for better health outcomes, as well as operational improvements in the value, efficiency, and/or effectiveness of services
Patient-Centered Medical Home
Participating health centers will commit to working with CHCANYS throughout the three-year project period to accomplish the HCCN project goals, as evidenced by executed Memoranda of Agreement to be provided upon request. Planning of specific grant activities is currently underway by CHCANYS staff and members of the current NYS-HCCN Committees.
We will develop a competitive proposal by extending and expanding on the NYS-HCCN’s existing governance and TA infrastructure, while leveraging CHCANYS’ highly respected statewide technology and quality improvement programs, the strengths of the community health centers in the state of New York, and the proven success of the Center for Primary Care Informatics (CPCI).
