The New York State Health Foundation’s “Best of 2012” Report, released in January, highlights a broad range of the Foundation’s initiatives and activities, including investments in Advancing Primary Care and Growing Community Health Centers. The report features NYSHealth’s projects with Refuah Health Center and Institute for Family Health related to primary care delivery and payment reform, grants to 16 community health centers in nine regions of the state to enable them to serve more patients, open new sites, or add new services, and NYSHealth’s investment in CHCANYS’ work to support New Access Point applicants, which resulted in 11 of the 12 applicants receiving awards potentially worth more than $25 million.
