Amy Freiman comes to CHCANYS with a background in practice operations and population health management. As a member of the Health IT team within CHCANYS' Quality and Technology Initiatives (CEI) department, Amy provides technical assistance to FQHCs statewide on the Meaningful Use EHR Incentive Program. She also supports the CPCI data warehouse program around customer service, training and adoption. In her prior work at the Primary Care Information Project within the NYC Department of Health and Mental Hygiene, Amy provided technical assistance to small primary care practices in underserved areas seeking to comply with the Meaningful Use EHR Incentive Program and achieve PCMH. Amy holds a Master of Public Health degree in Health Policy and Management from Columbia University and has been certified in PCMH CCE (Certified Content Expert) and Practice Facilitation.
