The level of interest in the recently released New Access Point opportunity is unprecedented. News from Clinical Quality Initiatives New Additions to Clinical Quality Initiatives Team Kameron L. Wells, ND, RN has joined the staff as the new Vice President for Clinical Initiatives for CHCANYS. Dr. Wells has been a healthcare professional for more than 25 years, beginning his career as a critical care nurse and ultimately receiving a Doctorate in Naturopathic Medicine from Bastyr University in Seattle.
As Vice President for Clinical Initiatives, Dr. Wells will have primary responsibility for directing and managing all current clinical improvement projects of CHCANYS as well as identifying and developing new opportunities for CHCANYS and health centers.
Dr. Wells comes to CHCANYS from Community Healthcare Network (CHN) in New York City, where he most recently served as AVP of Ambulatory Care Compliance/QI/Medical Informatics. In this capacity, Dr. Wells was responsible for supervising and evaluating the EMR system (eClinicalWorks) from a clinical standpoint; working with senior leaders and managerial staff in acquiring and maintaining Joint Commission accreditation status; overseeing organizational, departmental and programmatic Quality Improvement activities and the agency’s annual QI evaluation; and overseeing the development, implementation and quality management of CHN’s Medical Informatics program, products and services to address both the clinical needs of patients and training needs of clinical staff.
Prior to his position with CHN, Dr. Wells was a Naturopathic Doctor with The Atkins Center for Complementary Care in Manhattan, where he treated patients with acute and chronic conditions using herbal medicine, homeopathy, therapeutic nutrition and counseling.
Surkhab Peerzada, MPH has joined the CQI team as a Clinical Quality Initiatives Coordinator. In this capacity, Ms. Peerzada will be co-leading with Kathy Alexis, Clinical Quality Initiatives Manager, on CHCANYS’ Diabetes Campaign and new Depression Collaborative as well as the many endeavors that the CQI team will be undertaking.
Prior to joining CHCANYS, Ms. Peerzada received a Bachelors of Health Science degree with a minor in International Development from the University of Western Ontario. She recently graduated from New York University with a Master of Public Health degree with a specialization in International Community Health. Ms. Peerzada has more than four years of professional experience in project management and more recently conducted outreach to faith-based organizations for the Bronx Knows Initiative at the New York City Department of Health and Mental Hygiene – recruiting organizations and leaders to raise HIV/AIDS Awareness and Testing in the Bronx. She has worked in both developing and developed countries, independently and as a member of a diverse team. During her professional career thus far she has developed instruments to collect qualitative and quantitative data and collected as well as interpreted data using SPSS and Excel.

Current Projects CHCANYS Emergency Preparedness Program has begun recruiting for initiatives aimed at assisting our member health centers with their emergency management and planning needs. These initiatives will ensure that our membership can achieve the level of preparedness required by HRSA and Joint Commission operational standards for FQHCs.
NEW YORK CITY EMP Assistance* – participating sites will undergo an on-site evaluation by the CHCANYS EP staff to review their existing EMPs, to assess their quality and identify areas of improvement to ensure they meet acceptable standards. BCP Assistance* – CHCANYS EP staff will provide on-site assistance to health centers in creating or updating their Business Impact Analysis, reviewing their Business Continuity Plans, and developing an After-Action Report/Improvement Plan to ensure they meet all standards. Exercise Plan Templates – CHCANYS EP staff will draft 5 Emergency Preparedness Exercise Plans, the completion of which will help health centers satisfy drill requirements for Joint Commission and HRSA. We’re still looking for your input on what kinds of exercise plan scenarios would be best for your centers.
UPSTATE EMP Assistance* – participating sites will undergo an on-site evaluation by the CHCANYS EP staff to review their existing EMPs, to assess their quality and identify areas of improvement to ensure acceptable standards are met. EMP Template – CHCANYS will solicit feedback from centers to determine areas of improvement needed to update the Community Health Center Emergency Preparedness Manual Template. NYSDOH Communications Drill –In the coming year, NYSDOH is preparing two communication drills planned to improve emergency communications between health centers and the NYSDOH. CHCANYS will provide coordination between NYSDOH and our member health centers.

Management & Finance Amendments to PIN 2009-06 FQHC Look-Alike Guidelines and Application The Health Resources & Services Administration (HRSA) has released Policy Information Notice (PIN) 2010-02, "Amendment to PIN 2009-06, Federally Qualified Health Center (FQHC) Look-Alike Guidelines and Application, Additional Submission Requirements for Change in Scope Applications," to announce supplemental requirements for submitting change in scope (CIS) applications. Effective September 8, 2010, all FQHC Look-Alikes must submit CIS applications electronically through HRSA's Electronic HandBook (EHB) system. When completing CIS applications in HRSA's EHB, FQHC Look-Alikes must complete two additional forms, which were not included in PIN 2009-06. These forms are SF-424 Face Page and SF-424A Budget Information for Non-Construction Programs. PIN 2010-02 also discusses factors that HRSA considers in making determinations on FQHC Look-Alike CIS applications to add specialty services to the HRSA-approved scope of project.
