NYS Diabetes Campaign Launch New Website On September 21, 2009, the New York State Health Foundation announced the launch of a Diabetes Clinical Improvement Network as part of the efforts of the NYS Diabetes Campaign. For the first time, key clinical associations come together to improve outcomes for New Yorkers with diabetes across all types of primary care settings in NYS. Click here to learn more.

Enlist Your Health Center The NYS Diabetes Campaign, sponsored by the New York State Health Foundation (NYSHealth) in partnership with the Institute for Family Health, is an initiative designed to reverse the diabetes epidemic in New York State by improving clinical care, mobilizing communities, and promoting policy. CHCANYS has been chosen as one of the clinical hubs to participate in this effort and help primary care providers across New York State achieve high standards of care and improved patient outcomes. CHCANYS is now enlisting health centers to participate in the campaign.
The Campaign and CHCANYS will work with providers at whatever level they are ready to participate and assist health centers in applying for national diabetes recognition programs (National Committee for Quality Assurance\'s (NCQA) Diabetes Recognition Program and Bridges To Excellence\'s Diabetes Care Link program). H1N1 Update CHCANYS continues to assist both the NYS Department of Health and the NYC Department of Health and Mental Hygiene reach out to NY health centers to deal with what is projected to be a busy 2009 - 2010 flu season because of H1N1.
New York State: Vaccine Ordering Center Taking H1N1 Vaccine Orders for Upstate Sites The New York State Department of Health has released information on how upstate healthcare providers can order the 2009 H1N1 vaccine (NYC sites have a different method). The Bureau of Immunization\'s Vaccine Ordering Center is now accepting telephone calls for vaccine orders. The hours of operation for the toll-free number (1-800-543-7468) are Monday through Friday, 8:30 a.m. through 4:45 p.m., excluding state holidays and weekends.
Registration Forms for H1N1 Vaccine Now Available! The New York State Department of Health has released the pre-registration forms for becoming a distributor of H1N1 vaccine when it becomes available. The forms are available for both pediatric and adult providers. Click here to access the forms.

New York City: H1N1 Vaccine Ordering Info for NYC Sites 2009 H1N1 Vaccine can be ordered through the Citywide Immunization Registry (CIR). NYC Flu Diagnostic and Treatment Center Locator to be Launched Soon! The influenza services locator will be used by DOHMH to assist the public in finding a provider for vaccinations and non-acute treatment for influenza like illness. The locator, posted on the DOHMH web portal and accessible through 311, will allow individuals to search for services by zip code or borough. Provision of this locator by the City is intended to redirect health care surge related to this year\'s flu season away from hospital emergency departments and spread it as evenly as possible among primary care providers. DOHMH is anticipating the launching of the site on Friday, October 2nd.
Influenza Working Group Continues to Inform City Planners The Primary Care Influenza Working Group, led by CHCANYS EP Team and Dr. Bindy Crouch of DOHMH\'s Healthcare Emergency Preparedness Program (HEPP) continues to raise important issues for this coming flu season related to FTCA coverage and reimbursement. These questions, such as how will health centers be compensated for providing flu treatment for non-patients, continue to be filtered up through the appropriate channels at the State and Federal level. More updates will be available on the next Influenza Working Group conference call, 8:30 AM, Friday, October 2nd. Chronic Disease and Diabetes Self-Management Program Interested in offering chronic disease self-management group workshops but don\'t have the people to do it? Establishing lay leaders in the community is important when working with those who have chronic diseases. The Quality Initiatives team at CHCANYS has been trained to be Master Trainers of the Stanford Chronic Disease Self-Management Program and will be available to lead 4-day peer leader trainings for staff and/or community champions.
Now offering Diabetes Self-Management workshops. This program gives patients with diabetes and their caretakers the skills needed to manage their health and to maintain a healthy and active life.
