Enlist Your Health Center The NYS Diabetes Campaign, sponsored by the New York State Health Foundation (NYSHealth) in partnership with the Institute for Family Health, is an initiative designed to reverse the diabetes epidemic in New York State by improving clinical care, mobilizing communities, and promoting policy. CHCANYS has been chosen as one of the clinical hubs to participate in this effort and help primary care providers across New York State achieve high standards of care and improved patient outcomes. CHCANYS is now enlisting health centers to participate in the campaign.
The Campaign and CHCANYS will work with providers at whatever level they are ready to participate and assist health centers in applying for national diabetes recognition programs (National Committee for Quality Assurance's (NCQA) Diabetes Recognition Program and Bridges To Excellence's Diabetes Care Link program). Diabetes Policy Brief Release by NYS Health Foundation and Bridges To Excellence Now Available The New York State Health Foundation's Diabetes Policy Center, in partnership with Bridges To Excellence, has released their policy brief, which describes their new Diabetes Incentive Program Endorsement - strategy created to promote incentives that drive excellence in diabetes care and outcomes. The Center's goals are to encourage the majority of health plans in New York State to offer and deliver incentives to physicians and practices that demonstrate delivering quality care and achieving better outcomes for patients with diabetes and to encourage the alignment of incentive programs across New York's health plans. FREE Webinar - "Who Needs CLAS?" On Thursday, October 8th, 2009, the NYS Department of Health Bureau of Community Chronic Disease Prevention, Bureau of Chronic Disease Control, and Bureau of Injury Prevention invite public health and traffic safety professionals to attend a video-teleconference for a free, one-hour training entitled "Who Needs CLAS?" from 11:00 am to Noon. This training will focus on the four federally mandated Culturally and Linguistically Appropriate Services (CLAS) Standards and how they apply to your organization.
We are currently looking for sites to serve as a host for this video teleconference. In order to serve as a host, all your organization needs is video teleconference equipment that can be accessed by either an ISDN phone line or an IP address (check with your tech folks if you are not sure). If you are willing to act as a host site please send a note to Eric Weiskopf at esw01@health.state.ny.us by Wednesday, August 26th along with the following information:
Your name/address/contact info Name/contact info for technical point of contact (POC) Name of organization Name/address of room with video teleconference equipment Number of people you can accommodate in room IP address or ISDN number for equipment (Tech POC should know this) Are you willing to host guests from other organizations? A registration form will be sent out on Monday, August 31st. It will include the name and address of all host sites.
New Technical Assistance Materials on ARRA Reporting Now Available The federal government has posted on the main recovery website new technical assistance materials to assist grantees in submitting their ARRA government-wide reports. Further, on August 17, 2009, www.federalreporting.gov will go live and recipients of ARRA funds will be able to register as the first step in the reporting process mandated by ARRA. ARRA-funded Loan Repayment Offer for Health Professions Workforce Now Available Nearly $200 million in ARRA funding is now available to support student loan repayments for primary care medical, dental and mental health clinicians who want to work at National Health Services Corps sites. The purpose of the loan repayments is to bolster and expand the health professions workforce. This opportunity is available to those with and without high HPSA scores. The deadline for this opportunity is September 30, 2009 and applications will be accepted until that deadline or until available funds are expended. For details, click here.

The curriculum focuses on identifying patients at increased risk for colorectal cancer and improving screening rates utilizing the most recent guidelines and recommendations.

Chronic Disease and Diabetes Self-Management Program Have you seen the Surgeon General's Perspectives in this month's edition of Public Health Reports: "SELF-MANAGEMENT PROGRAMS: ONE WAY TO PROMOTE HEALTHY AGING." It mentions the role of programs such as Stanford University School of Medicine's Chronic Disease Self- Management Program in helping patients control their symptoms and manage their chronic illness.
The Quality Initiatives team at CHCANYS has been trained to be Master Trainers of the Stanford Chronic Disease and Diabetes Self-Management Program. Staff is available to facilitate a 6-week session for identified patients of your facility. This program gives patients with chronic diseases and their caretakers the skills needed to manage their health and to maintain a healthy and active life.
Kudos to Whitney M. Young, Jr. Health Services Whitney M. Young, Jr. Health Services has taken the initiative to host the Stanford Chronic Disease Self-Management Program (CDSMP) workshop for their patients and caretakers. In addition to supporting facilitators from CHCANYS, WMY was successful in recruiting participants and allotting the resources needed to make the trainings possible. After an engaging and exciting 6-week series, the workshop resulted in 15 participants graduating from the CDSMP. "I suffer from depression but since taking this class I haven't been depressed. I've had something to look forward to." - WMY CDSMP graduate. Thank you to WMY patients and staff for making this an enjoyable experience.
