Register Now for Population Health Summit: Making New York the Healthiest State - Achieving the Triple Aim New York State Health Commissioner, Nirav R. Shah, M.D., M.P.H. will convene a population health summit on December 3, 2013 to bring together state and national leaders to learn about the role public health and other sectors play in improving population health. Capital Needs Assessment Deadline is November 12! On October 16, Capital Link sent an online assessment to all health centers. This important study has been designed to determine existing and future capital requirements for health centers nationwide and will provide an estimate of the need for additional capital investment so health centers and primary care associations can plan for growth. The data collected will assist Capital Link and CHCANYS in identifying strategies for securing capital funding to help health centers achieve their expansion goals. read more >>

NYSHF Practitioner Database Project: Your Feedback Requested The New York State Health Foundation (NYSHF) has funded HealtheConnections to develop a plan for a statewide practitioner database, covering physicians and ‘mid-level’ practitioners. The Practitioner Database Project involves a wide range of stakeholders, including potential organizational members ofCHCANYS, but the project staff envisions a product that could appeal to local agencies and the general public as well. read more >>

Outreach & Enrollment Update Thanks to all who attended Getting Our Patients Covered: What the New Exchange Has to Offer, the special training session for Outreach and Enrollment (O&E) staff presented by Trilby de Jung of Empire Justice Center on October 29th in Saratoga Springs. Although there were a few technological glitches, the training was a great success overall, both in terms of sharing best practices as well as learning about issues that still need to be addressed. read more >>
