Final MRT Report to Serve as Framework for 1115 Superwaiver Application The final report of the Medicaid Redesign Team (MRT), released on May 3, 2012, nicely summarizes both the accomplishments of the State and the vision for the health care delivery system going forward. The report will also serve as the basis of the State’s application for an 1115 “Superwaiver” which, if granted, will redirect $10 billion of the $34 billion in State and federal savings resulting from the MRT reforms to allow for their full implementation. The State will issue a draft proposal in June for public comment and will submit the final version of the waiver application in November 2012.

FQHC Resource Page CHCANYS is pleased to announce the launch of the FQHC Resource Page, a dynamic repository of information and links designed to help both existing FQHCs and organizations in the planning stages to become and remain compliant with the Health Resources and Services Administration’s (HRSA) 19 Health Center Program Requirements--conditions that all FQHCs must meet in order to open and remain in operation and that are essential to maintaining strong organizations with the capacity to provide high-quality, cost-effective and accessible care.
The FQHC Resource Page is housed on the CHCANYS’ website. Intuitive and easy to navigate, the page has been organized into the same four main categories as are the HRSA Program Requirements. Each category features topic-specific links to tools and information. The page also includes a “General Resources and Guides Section” with links to the Program Requirements, existing new start guides, and other helpful overviews that are available online.

Eligibility: Eligible applicants must: (a) Be a school-based health center or a sponsoring facility of a school-based health center as defined in 4101(a)(6) of the Affordable Care Act, as set forth in section 2110(c)(9) of the Social Security Act (42 USC 1397jj(c)(9)). (b) Provide assurance that funds awarded will not be used to provide any service that is not authorized or allowable by federal, state or local law (such as supporting space which will be used for activities inconsistent with the operations of the school-based health center and/or rented to other entities for the purpose of generating revenue). Applicants for the SBHCC award must be operational within the two-year budget period and must administer and report on the use of federal funds. Summary: For FY 2013, the SBHCC awards will address significant and pressing capital needs to improve service delivery and support the expansion of services at school-based health centers (SBHCs) through the alteration/renovation of an existing facility(ies), construction of a new site(s) or expansion of an existing site(s), and/or the purchase of moveable equipment. Applicants must demonstrate how their proposal will support the SBHC in providing more effective, efficient and quality health care and will lead to improvements in access to health services for children at a SBHC.
An eligible applicant may submit one application proposing a maximum of 5 projects including any combination of site-specific, stand-alone facility projects and one project to purchase moveable equipment for multiple SBHC sites. Applicants may not request SBHCC funds to support the provision of health services, personnel or other operational expenses. For FY 2013, applications for SBHCC funding will propose to address capital needs at school-based health centers (SBHCs), through the following types of projects:
Alteration/renovation (A&R) (existing facility): Project types include: acquisition of an existing facility; associated work required to modernize, improve, and/or reconfigure the interior arrangements or other physical characteristics of a facility; and/or installation of permanently affixed equipment in an existing facility. This type of project would not increase the total square footage within an existing building. This type of project may include the purchase of related moveable equipment. Furniture, administrative equipment (i.e., computers, servers, telephones, fax machines, copying machines, software) and special purpose equipment used for medical activities (e.g., stethoscopes, blood pressure monitors, scales, electronic thermometers) with a useful life of one year or greater and a unit cost of less than $5,000 may also be included.
Construction (new site or expansion of existing site): Project types include: construction of a new stand-alone structure; acquisition or improvement of land or an existing facility; and/or associated work required to expand a structure to increase the total square feet of the facility. Construction projects may include use of a permanently affixed modular or prefabricated building and/or the purchase of related moveable equipment. Furniture, administrative equipment (i.e., computers, servers, telephones, fax machines, copying machines, software) and special purpose equipment used for medical activities (e.g., stethoscopes, blood pressure monitors, scales, electronic thermometers) with a useful life of one year or greater and a unit cost of less than $5,000 may also be included.
Equipment-only projects: This project type includes the purchase of moveable equipment, including mobile vans. Furniture, administrative equipment (i.e., computers, servers, telephones, fax machines, copying machines, software) and special purpose equipment used for medical activities with a useful life of one year or greater and a unit cost of less than $5,000 (e.g., stethoscopes, blood pressure monitors, scales, electronic thermometers) may also be included.
A funding preference will be used for this announcement. In order to be eligible for the funding preference, applicants must certify that the SBHC site(s) serves or will serve a large population of children eligible for medical assistance under the state Medicaid plan under title XIX of the Social Security Act (the Act) or under a waiver of such plan or children eligible for child health assistance under the state child health plan under title XXI of the Act (42 U.S.C. 1397aa et seq.).

CHCANYS’ Diabetes Learning Series – Diabetes Prevention: A Clinically Sound and Community Partnered Approach One in four adults in the US has pre-diabetes. Nearly all will develop diabetes, unless they lose weight and become more physically active. In fact, one in two African American and Latino children will develop diabetes unless our lifestyles change. Clinicians play a key role in diagnosing and managing pre-diabetes. Yet, our patients live in environments that promote obesity and sedentary activity. Thus, clinicians need to understand the social determinants of obesity and diabetes and discover community-based assets to help their patients in the struggle to be healthy. This webinar will describe the diabetes epidemic, strategies to prevent diabetes, and highlight successful and promising community-academic partnered diabetes prevention efforts.

The NYS Diabetes Campaign The National Committee for Quality Assurance and the Bridges to Excellence (BTE) Diabetes Recognition Program (DRP) are voluntary programs for practices to receive recognition for delivering quality diabetes care. Recognized health centers enjoy benefits such as being acknowledged as leaders in the community of practice and leveraging NCQA DRP data abstraction efforts for incentives such as that received through the Patient-Centered Medical Home (PCMH) Recognition. CHCANYS staff is available to provide technical assistance to those submitting for these recognition programs and to explore opportunities to improve clinical care outcomes.

Your Updates Needed: 2011-2012 Critical Asset Survey for Community Health Centers The New York State Department of Health (NYSDOH) is requesting that all health centers update their information on the 2011-2012 Critical Asset Survey for Community Health Centers. Your Health Center’s HCS Director/Coordinator(s) need to either complete or update the survey annually. For those of you with multiple access points, a survey must be completed for each site. Although many of you completed this survey last year, the NYSDOH is asking that you again access the survey, verify the information that appears, and resubmit it so that the Health Commerce System is up to date with the latest information.
CHCANYS will provide outreach to assist the NYSDOH in tracking participation and the progress of the survey responses. The survey results will provide the NYSDOH with important information regarding your needs in the event of a catastrophic accident, disaster, or health emergency and will allow the NYSDOH to plan for the deployment of emergency supplies and medicine to your center prior to or during an emergency.
