CHCANYS is committed to maintaining, strengthening and expanding the existing and potential FQHCs in New York State. Our work is focused on ensuring that FQHC services and locations are accessible and meet the needs of a large and growing unserved / underserved population. Through its Health Center Support Program, CHCANYS assists existing FQHCs and new-starts as well as those applying or planning to establish FQHCs in their communities.
Community-based primary care providers, including Federally Qualified Health Centers, have been recognized as a crucial centerpiece of any health care reform strategy at both the federal and state levels. Expanding the capacity of community-based primary care to serve more patients is an important step toward achieving the Triple Aim of better care and better health at lower costs. The resources below have been developed to help strengthen existing health centers as well as assist those interested in developing new ones.
A Plan for Expanding Sustainable Community Health Centers in New York Developed by CHCANYS with support from the New York State Health Foundation, this report details a statewide plan for community health centers designed to increase their ability to serve more patients. Based on extensive quantitative and qualitative analyses, the plan identifies geographic areas that have the greatest need and potential for sustainable growth, estimates potential increases in capacity within the existing system, and highlights strategies for creating more capacity.
