QUALITY IMPROVEMENT

This webinar, led by CHCANYS’ Co-Directors of Data Systems, Jeff Barnes and James Welsh, will briefly introduce the following four data tools recently distributed to NYS health centers by CHCANYS:

1. Map Atlas: a collection of 22 maps showing concentrations of social determinants, patient needs, and patient populations within your service area
2. Community Resource Guide: a spreadsheet tool that identifies health facilities, behavioral health programs, adult care, WIC, supportive housing, and other social service programs in your service area
3. ED usage and Hospitalization Data: a compendium of regional data on ED use, hospitalizations and high-usage patient groups
4. Clinical Comparison Data: a summary of quality and clinical measures for your health center compared to peer groups of similar centers
